Change of name on an existing British Passport
How do I apply?
You should submit the following documentation (only original documents please, photocopies are not acceptable)
- A completed passport application form:
- C1 if you are 16 years of age and over. Please also read guidance notes or
- C2 if you are under 16 years. Please also read guidance notes or
- BNO A (Hong Kong nationals) if you are 16 years of age and over. Please also read guidance notes, or
- BNO B (Hong Kong natioanls) if you are under 16 years of age. Please also read guidance notes.
- Existing British passport. Expired passports will be cancelled and returned to you.
- Two identical passport size photographs - one photo must be countersigned by the person who countersigns your application form. The photograph standards required for biometric passports are higher than those needed for earlier types of passports. If the photograph does not meet the standards required your complete application will be returned to you at your expense. This could cause a long delay in processing your application. - If you have changed your name by marriage we require your marriage certificate issued by the Registrar of Births Deaths and Marriages.* Please pay attention to the correct marriage certificate.
- If you are reverting to your maiden name you will need to forward your original birth certificate.
- If you have changed your name by Deed Poll you will need to forward the original Deed Poll.
- If you have changed your name by usage only, we require a completed statutory declaration witnessed by a Justice of the Peace together with two forms of identification showing your new name e.g. a photocopy of your driver's licence, Medicare card etc. Statutory Declaration forms are available from newsagents.
- A signed checklist confirming you have enclosed all the required documentation.
NOTE
* Marriage certificates issued by the Church or Celebrant on the wedding day are not acceptable. Divorce documents are not acceptable. Australian marriage certificates are available from the Registrar of Births Deaths and Marriages in the State in which the marriage took place.
