Lost or stolen passports
This section applies to those whose valid passports have been lost or stolen. However, if your lost/stolen passport had expired you should submit the original documents required for a first time issue. (Please see how to apply section to see what category applies to you).
To replace your passport you should submit the following documentation (only original documents please, photocopies are not acceptable).
- A completed passport application form:
- C1 if you are 16 years of age and over. Please also read guidance notes or
- C2 if you are under 16 years. Please also read guidance notes.
- Passport fee (we are unable to accept personal cheques, American Express or Diners Club).
- Two identical passport size photographs - one photo must be countersigned by the person who countersigns your application form. The photograph standards required for biometric passports are higher than those needed for earlier types of passports. If the photograph does not meet the standards required your complete application will be returned to you at your expense. This could cause a long delay in processing your application. - LSO1 (Lost/stolen notification form).
- Police report and reference number.
- Two forms of identity eg. your birth certificate, driving licence (photocopy acceptable), Medicare card (photocopy acceptable), original bank or credit card statement, gas, telephone or electricity bill.
- A signed checklist confirming you have enclosed all the required documentation.
