First time applicants are required to provide full original documentation together with a photocopy of each document.
What documents does a child need to submit in order to apply in this category?
The parent of the child should submit the following documentation on behalf of the child:
- A completed C2 passport application form. Please also read guidance notes.
- Passport fee sheet and passport fee; (we are unable to accept personal cheques, American Express, Diners Club or BankCard).
- Two identical passport size photographs of the child - one photo must be countersigned by the person who countersigns your application form. The photograph standards required for biometric passports are higher than those needed for earlier types of passports. If the photograph does not meet the standards required your complete application will be returned to you at your expense. This could cause a long delay in processing your application. (See links on right of page for standards of photographs required and also a suggested list of photographers)
- The child's full birth certificate showing both parents' names.
- Father's original birth certificate* showing his parents' names.
- One form of official photo identification showing father's name (eg: photocopy (not original) of passport or driving licence) or two other forms of identification (eg: photocopy (not original) of your Medicare card or an original (plus photocopy) of bank, credit card statement, gas, telephone or electricity bill).
- One form of official photo identification for the child (eg: original (plus photocopy) of Australian passport) or two forms of identification (eg: photocopy (not original) of Medicare card showing child's name and a letter from a doctor or an immunisation certificate).
- One form of official photo identification for the mother (eg: photocopy (not original) of passport or driving licence) or two other forms of identification (eg: photocopy (not original) of your Medicare card or an original (plus photocopy) of bank, credit card statement, gas, telephone or electricity bill) or mother's original (plus photocopy) of birth certificate.*
