Do not send in your application until you have all the correct documents. All documents must be the originals
· BNO-A application form and guidance notes
· BNO-B (under 16 years) application form and guidance notes
· Passport fee sheet and fee (Cheques, American Express and Diners Club are not accepted)
· Existing BNO passport
· Three passport size photographs - one photo must be countersigned by the person who countersigns your application form. The photograph standards required for biometric passports are higher than those needed for earlier types of passports. If the photograph does not meet the standards required your complete application will be returned to you at your expense. This could cause a long delay in processing your application. (See links on right of page for standards of photographs required and also a suggested list of photographers)
· If your name has been changed by marriage we require a certificate issued by the Registrar of Births Deaths and Marriages.* Please pay attention to the correct marriage certificate.
If you are reverting to your maiden name forward your original birth certificate.**
If you have changed your name by Deed Poll forward the original document.
If you have changed your name by usage only, we require a completed statutory declaration witnessed by a Justice of the Peace together with two forms of identification showing your new name e.g. a photocopy of your driver's licence, Medicare card etc. Statutory Declaration forms are available from newsagents.
Notes
* Marriage certificates issued by the Church or Celebrant on the wedding day are not acceptable. Divorce documents are not acceptable. Australian marriage certificates are available from the Registrar of Births Deaths and Marriages in the State in which the marriage took place.

The application and photos should be countersigned by a professional, British or Commonwealth (includes Australian) citizen who has known you for at least TWO years. Relatives must not countersign. If your child is under 16, the person who countersigns the application form must have known the parent for the required time, not the child.
Applications forms should be completed in black ink and must be signed within the box on the BNO-A or BNO-B. Failure to do this will delay your application.
